I have taken an extended medical leave of absence, and cannot be certain of my date of return to practicing medicine... I enjoyed my years of service to my patients and my community. Thank you for entrusting me with your medical care... You will need to make arrangements to find another physician to provide your medical care. You may find another physician from the community by contacting your health insurance company or consulting with your primary care provider.
