Concorde Health works closely with Age UK's Hillingdon Primary Care Navigator scheme. This has contributed significantly to the health and well-being of patients in the London Borough of Hillingdon by linking them to a range of community services and reducing the need for unplanned care... Concorde Health consists of four NHS GP Practices. We are all based in Hillingdon, Uxbridge, Middlesex. We have come together and formed our own Network. We hope to provide a better range of services to our patients as a Network.
