New patients: Please arrive 10 minutes early to fill out paperwork if unable to print out online forms. You may fill out in car depending on what current CDC guidelines state regarding the pandemic. Regarding x-rays, our office policy is that all new patients need diagnostic x-rays to be seen. We can take them here or you may have them forwarded from your previous office on or before your first visit as long as they are less than 2 years.
