Our staff can assist you with transferring your file from another pharmacy, or contacting your physician for refills. You can also print a copy of the form and fax/mail to the My Meds by Mail Program at 1-855-821-0950... Metro Ontario Pharmacies Limited operates 74 pharmacies across Ontario. Participants can have their prescriptions delivered directly to their homes or site of choice.
