MOHAWK VALLEY BEHAVIORAL HEALTH CARE COLLABORATIVE

The goal of our network, through cross-systems partnerships, is to ensure that individuals in our region have access to well-coordinated, high quality community-based mental health and substance use services and supports, prevention services, and other critical services like housing and food supports that fundamentally impact health and wellness. By focusing on delivering evidence based interventions that are data driven and patient-centered, our network will reduce costs to the healthcare system while improving quality of care.
