Foundations Behavioral Health, LLC is a certified provider for Medicare and Medicaid (SoonerCare). For these clients, we do not collect a co-pay, so there are no out-of-pocket costs for our services. Our clinicians go to nursing homes and retirement homes to provide confidential behavioral health services to geriatric clients. Counseling assists older adults adjust to the transitions of aging. We also provide family and caregiver counseling. All in-home counseling is private and confidential...
