Our goal is to ensure that children, adults, and seniors with disabling conditions and chronic illnesses in need of long-term care have a choice, control and access to a full range of quality services that ensure optimal outcomes, such as independence, health, and quality of life... MCCN is an affiliation of lead agency Seven Hills Foundation, BAMSI and Advocates, along with a strong network of agencies with expertise in supporting people with complex LTSS needs. Using our deep knowledge and understanding of people with developmental and intellectual disabilities, brain injuries, physical disabilities and complex medical needs we provide exceptional care coordination services for MassHealth members enrolled with our ACO Partners.
