Pathfinders-Care provides a home for adults of working age with complex physical nursing rehabilitation needs. Care is provided 24 hours a day, 7 days a week by multi disciplinary staff group. Our high staffing levels and a high degree of training equips us for caring for people with complex physical care and support requirements. All staff are trained to meet individual needs and work is constantly reviewed to ensure the highest level of support. We have an acknowledged low turnover of staff and low absence levels leading to high levels of consistency of care this is further embedded with key worker and key nurse systems for continuity and personalised care... We specialise in assisting people with long term conditions and co-morbidities, progressive conditions, end of life care, trauma, re-enablement and rehabilitation. Providing and promoting quality of life, choice, dignity and independence wherever possible... We provide excellent specialist care for people with complex care..
