Elizabeth Siraco

Dr. Siraco has practiced family medicine for the last 25 years- all within the Milford Regional Healthcare System. As Milford Regional Physician Group's Medical Director for the past six years, she has successfully and courageously led the group through not only day-to-day patient care issues, but also a number of significant challenges, including Epic (electronic medical record) implementation and the Covid-19 pandemic. In addition to her Medical Director role, Dr. Siraco continues to provide direct patient care through her practice at Blackstone Valley Family Physicians in Northbridge. Dr. Siraco's strong leadership was never felt more than during the Covid-19 pandemic. Serving as a conduit of information for providers, staff and patients, she worked in tandem with MRPG's infectious disease providers to continuously revise the system's employee health guidelines and provide up-to-date information regarding treatment, testing, and immunization protocols and availability. Dr. Siraco received her undergraduate degree with honors from Wellesley College. She briefly worked at Eli Lily as a pharmaceutical rep before heading off to UMass Medical School to earn her medical degree. Soon after completing her residency in family medicine, Dr. Siraco joined MRPG and was the first family medicine physician to provide obstetrics care at Milford Regional Medical Center. She is currently Chair of Family Medicine and Med/Peds at MRMC, Co-Chair of the MRPG EMR Committee, and chair of MRPG Peer Review Committee.

Kayt Kitt

As our Director of Population Health, Training and EMR Services, Kayt is responsible for the Population Health, Training, and Referral Management teams as well as EMR use and optimization across the organization.Prior to this role, Kayt has served as MRPG's Population Health Programs Administrator for three years. She has been instrumental in not only supporting the needs of our population health team including training, reporting and workflow development, but also in providing guidance to practitioners and staff regarding referral orders, telehealth, and iPad questionnaires, and serving as the administrative lead on our Epic Task Force. Prior to coming to Milford Regional Physician Group, Kayt held a variety of roles in population health at MGB, formerly known as Partners Healthcare, starting out as a Project Specialist and working her way up to an iCMP Program Manager. She also spent some time at Mass General Hospital and Newton-Wellesley Hospital. Kayt earned her Bachelor of Science and Master's Degrees from Suffolk University. She currently lives in Medway with her husband and two children.

Nancy Jolicoeur - COO

Nancy has been a member of the Milford Regional Healthcare System since 1987. During that time, she held several leadership positions both at the Hospital and at Milford Regional Physician Group. Nancy participated in the growth of our group from its infancy in 1992 where she supported the Benefits, Employee Relations, and Compensation functions. It was a natural progression for Nancy to join Milford Regional Physician Group in 2002 and develop the Human Resources Department. In 2007, Nancy became very involved with the overall operations of Milford Regional Physician Group's practice locations. She led the Administrative EMR implementation team and oversaw the billing and coding department, IT department as well as marketing and purchasing departments. Nancy, reporting to the President, is currently responsible for Physician Recruitment, Lease and Contract Development, Strategic Planning, Office Design Management, as well as Patient Satisfaction and Project Leadership. Nancy serves as Milford Regional Physician Group's Liaison for Partners Community Physicians Organization (PCPO). In this role she is responsible for analyzing, developing, and communicating workflows to assist our offices in meeting quality and efficiency targets and measures.

Patrick McSweeney - President

Patrick McSweeney, MD, joined Milford Regional Physician Group in 2000. In the years since, he has cared for thousands of patients in his Medway practice, Family Medicine Associates. In September 2019, Dr. McSweeney became President of MRPG. In this role, he is responsible for leadership, strategic planning, and growth and development of the group. A key component in this position is physician recruitment and retention. Dr. McSweeney spends much of his time working collaboratively with physicians to ensure that the group, and the Milford Regional Healthcare System overall, continues to be successful while delivering the highest quality care to patients. Dr. McSweeney is a member of the senior management team for the Milford Regional Healthcare System and works closely with the system's CEO on strategic decisions. Prior to his appointment as President, he served in numerous leadership roles for our organization, including as MRPG Medical Director for 12 years and a member of the Milford Regional Medical Center Board of Trustees for 9 years. This background as well as his experience working directly with patients helps to inform his decision-making. Dr. McSweeney graduated from Providence College and earned his medical degree from University of Massachusetts Medical School. He completed the Brown University Family Medicine residency program (at Memorial Hospital of Rhode Island) including serving as chief resident. At the start of his medical career, Dr. McSweeney worked for the Franklin Family Medicine Center as part of the Norwood/Caritas Medical Group. Combining his leadership abilities with clinical experience, Dr. McSweeney is committed to bringing quality healthcare to the local community. He grew up in Medway, where he lived for many years, until recently moving to Wrentham.

Rachel Kagno

Rachel joined Milford Regional Physician Group in August, 2020, during the height of the Covid-19 pandemic. She hit the ground running with internal and external communications to educate patients and the community at large about signs and symptoms of Covid-19, testing options, and the important benefits of getting the Covid vaccination. In her day to day role, she is responsible for the strategy and implementation of marketing, communications, and public relations activities for 130 providers and 43 practice sites, including management of website, intranet, social media, print and digital advertising, office signage, video production, media relations, and other marketing vehicles. Rachel has decades of experience working in the healthcare industry. Prior to Milford Regional Physician Group, she was the Media Relations Manager at MassGeneral Brigham-affiliated Newton-Wellesley Hospital for 23 years. While at NWH, she was honored to serve on a number of Boards in the community, including Newton at Home, a nonprofit organization dedicated to keeping older adults living in their homes safely and independently. In addition to serving for five years as an NAH Board Member, Rachel provided direct service by taking members to medical appointments, grocery shopping, and just about anywhere else they needed to go locally. Rachel's love for working with the senior population also led her to the Shillman House in Framingham, a 2Life independent living community. At Shillman, she worked as a "Helping Hand" at the front desk, assisting residents when needed and performing Covid testing for families and visitors. A resident of Holliston, she is the mother of two boys (now 23 and 19) and thrilled to be living and working in her own community.

Tammi Koppy

Tammi joined Milford Regional Physician Group in 2015 as a Project Manager in Operations. She assisted the management team in data analysis, marketing, management of payer contracts, and more. In the summer of 2018, she transitioned into the role of Director of Population Health. In that position, she was responsible for administration of programs aimed at supporting our providers in managing different populations of patients, including those at high risk based on their medical conditions, behavioral health and social service needs. Her team focused on eliminating barriers to care to ensure that patients are getting the care they need when they need it. Tammi became the Director of Financial & Contract Performance Management in 2020, continuing oversight of these programs and expanding her responsibilities to include producing reports to track site performance on a weekly and monthly basis, administering various provider compensation models, and development and monitoring of the group's annual operating budget. In 2023, Tammi transitioned to focus more on MRPG's financial success, incorporating oversight of the group's coding auditors and medical records teams, while relegating oversight of the clinical support programs. A graduate of the University of New Hampshire with a Master's Degree in Health Administration, Tammi has over 25 years of experience working in the healthcare industry. Prior to joining Milford Regional Physician Group, Tammi worked at Mass General Brigham (formerly Partners Healthcare) for more than 15 years in a variety of roles, including as Account Manager, overseeing the relationships and contracts with a number of affiliated organizations- including Milford Regional Physician Group.
