The coalition, which first came together in 2015 under the name Advance Care Planning: Quality Conversations, has now expanded its initial focus to provide mentoring on Palliative Care. This expansion has been a natural process because Advance Care Planning (ACP) is one of the three pillars of Palliative Care. Palliative care is an interprofessional approach to care that focuses on improving quality of life and reducing suffering among patients with life-threatening illness. It is supported by three major pillars: 1 - Identify and respect patient goals of care, 2- control distressing symptoms, and 3- offer holistic care through integration of physiological, psychological and spiritual domains.
