To enhance patient self-management and provider health literacy, partnering with primary care as well as community based health and social service providers, we offer evidence-based programs for community members and professional development training for health care providers... For community based programs, using an equity lens, we offer our services in partnership with 33 unique health and social service sector organizations to ensure that individuals are able to access self-management programs in proximity to where they live or work... We also offer professional development training for health care providers that are grounded in motivational interviewing and support rapport building skills. These half to full day training opportunities are intended to support providers in engaging their clients in discussion about self-management.
