Derek Freshman

Derek Freshman is a course instructor for Upstream Rehabilitation Institute. Derek obtained his Bachelors in Exercise Science from the University of South Carolina in 2008 and his Doctorate in Physical Therapy from Thomas Jefferson University in 2012. Derek currently practices in Mechanicsburg, Pennsylvania as a Senior Physical Therapist. Derek obtained his Board Certification in Orthopedics in 2017 and is seeking to complete Fellowship in Manual Therapy through the American Academy of Orthopedic Manual Physical Therapy. Derek also brings years of experience and training through the McKenzie/Mechanical Diagnosis and Therapy System as well as the Selective Functional Movement System. Derek is active in the Pennsylvania Chapter of the American Physical Therapy Association and mentors residents within the Upstream Rehabilitation Institute as well as serves as a Credentialed Clinical Instructor mentoring student Physical Therapists from all over the northeast.

Dr. Anjanette Nunez

Dr. Anjanette Nunez is a faculty member and course developer for Upstream Rehab Institute and is involved in the Modern Pain Science course. She received her Doctor of Physical Therapy degree from Georgia State University (GSU) in 2014. Dr. Nunez completed the BenchMark Rehab Institute Orthopaedic Physical Therapy Residency program in 2016, she is a board certified orthopaedic clinical specialist (OCS) and is certified with the International Spine and Pain Institute as a Therapeutic Pain Specialist (TPS). Dr. Nunez has presented research on Pain Neuroscience Education at national and state conferences. She is an active member of the APTA and serves on the 2017-2018 Nominating Committee for PTAG. Dr. Nunez is currently adjunct faculty at GSU where she received the Outstanding Clinical Instructor Award from GSU for 2017-2018 the academic year.

Dr. Ashlie Crewe

Dr. Crewe is a course developer and instructor with Upstream Rehab Institute and the Regional Coordinator of Pelvic Floor Rehab for Drayer Physical Therapy Institute in Hummelstown, PA. Ashlie earned her Doctor of Physical Therapy degree at Gannon University and she has completed Pelvic Floor Series and various other specialty courses from the Herman and Wallace Institute. She is board certified as a women's health clinical specialist (WCS) and is a Certified Ergonomic Assessment Specialist. She is a member of the International Pelvic Pain Society and the Global Pelvic Health Alliance. Dr. Crewe is active in teaching roles with Herman and Wallace, is an adjunct faculty member at Lebanon Valley College, and heads up a pelvic floor webinar education series for URI.

Dr. Barrett Stanley

Dr. Barrett Stanley is a course developer and instructor with Upstream Rehab Institute. He obtained his Doctor of Physical Therapy degree from Duke University and currently practices in Oakwood Georgia. Following graduation with his DPT he completed the Benchmark Rehab Institute's Orthopaedic Residency and became a board certified orthopaedic clinical specialist (OCS) and is currently pursuing completion of an Orthopaedic Manual Physical Therapy fellowship. Dr. Stanley is currently a researcher in a randomized controlled trial investigating the "Local and Widespread Hypoalgesic Effect Following Neurodynamic Mobilization". He has a wide-range of manual therapy experience which is bolstered with his knowledge of exercise science and prescription through his years of being a Certified Strength and Conditioning Specialist.

DR. BILL JOHNSON

Dr. Johnson is a course developer and lead instructor within the Upstream Rehab Institute (URI) as well as instructor/faculty in URI Orthopeadic and Sports Physical Therapy Residency programs. Dr. Johnson received a Master of Physical Therapy in 1994 and Doctorate in Physical Therapy in 2005 from Shenandoah University. He is a board certified clinical specialist in orthopedic physical therapy (OCS) and a certified Advanced Golf Fitness Instructor (AGFI). He has copy written programs for knee injury prevention for female athlete (S.K.I.P.©: Successful Knee Injury Prevention), and performance enhancement for speed, agility, quickness and power. Bill continues to be an active clinician working with patients and clients of all ages and abilities, as well and enjoys mentoring newly hired associates in his region.

Dr. Chris Myers

Dr. Chris Myers is the Director of Continuing Education and an instructor with Upstream Rehab Institute's Orthopaedic Physical Therapy Residency, OMPT Fellowship, and continuing education programs. She received her Master of Physical Therapy degree from Elon College, her Doctor of Physical Therapy degree from the University of Tennessee at Chattanooga and is currently a Doctor of Science in Physical Therapy candidate with Bellin College where she is researching the correlation between therapeutic alliance and outcomes as well as opioid use post-lumbar surgery. Dr. Myers completed fellowship training with Evidence in Motion and is a fellow of the American Academy of Orthopaedic Manual Physical Therapists. She is also a board certified clinical specialist in orthopedic physical therapy (OCS) and certified in the McKenzie Method of Mechanical Diagnosis and Therapy (Cert. MDT). Dr. Myers is the Assistant Director of Clinical Education and Assistant Professor for the Doctor of Physical Therapy program at South College in Knoxville, TN. She continues to present at both national and state level conferences annually as well as teaching numerous continuing education courses throughout the country.

Dr. Daniel Maddox

Dr. Daniel Maddox is the program director of Upstream Rehab Institute's Fellowship in Orthopaedic Manual Physical Therapy and is lead faculty for URI Orthopaedic Residency and continuing education courses. He earned his doctorate in physical therapy from the Medical College of Georgia, and he is currently pursuing his Doctor of Science (DSc) degree through Bellin College. Dr. Maddox completed OMPT Fellowship training with Evidence in Motion and is a fellow of the American Academy of Orthopaedic Manual Physical Therapists (FAAOMPT). He is also a board-certified clinical specialist in orthopedic physical therapy (OCS) and a certified Therapeutic Pain Specialist through the International Spine and Pain Institute. He remains an active member of the APTA and its Georgia chapter - where he currently serves as Awards Committee chair and has previously served as a Legislative Committee member, delegate, chief delegate, and board member. In addition, Dr. Maddox is currently chair of the AAOMPT Education Committee and is actively involved in AAOMPT's Program Director and Academic and Clinical Faculty Special Interest Groups. Dr. Maddox is currently an assistant professor in the Doctor of Physical Therapy program at Brenau University, and since 2009 has had adjunct faculty positions and guest speaker roles at multiple institutions - including Georgia State University, Augusta University, University of North Georgia, and South College. Dr. Maddox is engaged in research initiatives through both URI and Brenau. His research interests include the management of chronic and/or complex pain and exploration of the mechanisticeffects of various forms of manual intervention.

Dr. Derek Clewley

Dr. Clewley is an instructor with Upstream Rehab Institute's Ortho Residency, OMPT Fellowship, and continuing education programs. He is an educator, clinical researcher, physical therapist, and mentor in the Duke University DPT program. His specific area of clinical expertise and training is orthopaedics and manual physical therapy. Dr. Clewley has achieved board certification in orthopaedics and is recognized as a Fellow of the American Academy of Orthopaedic Manual Physical Therapists. He has a PhD with a primary interest in health services related research including the specific investigation of healthcare seeking behavior. In addition, his peer reviewed publication record includes the interventional topics of manual physical therapy, dry needling, and pain sciences. His publication record also includes clinical practice guidelines and has presented nationally and internationally on the use and implementation of guidelines in clinical practice. Dr. Clewley has extensive experience in the development of continuing education, residency, and fellowship post-graduate training programs.

Dr. Destiny Hebert

Dr. Destiny Hebert is a course developer and instructor with Upstream Rehab Institute. She received her Master of Science in Physical Therapy from North Georgia College & State University and her Doctor of Physical Therapy degree from the Medical College of Georgia. Dr. Hebert has completed certification and Diploma in Spinal Manipulation and Diploma of Osteopractic through the American Academy of Manipulative Therapy. She has also completed certifications in vestibular rehab, ASTYM, ISTYM, medical acupuncture and the STAR Cancer Program. Dr. Hebert completed an orthopaedic manual physical therapy fellowship with the American Manual Therapy Institute and obtained status as a fellow of the American Academy of Orthopaedic Manual Physical Therapists (FAAOMPT). She teaches multiple continuing education courses throughout the country each year and has been a guest lecture/adjunct faculty member for numerous Doctor of Physical Therapy programs. Dr. Hebert is an active area director and clinician for BenchMark Rehab Partners in the Atlanta area. She is currently working on research in the field of vestibular rehab and dry needling at the University of North Georgia where she holds a faculty appointment.

Dr. Domonique Martin

Dr. Domonique Martin is the Sports Residency Coordinator. She obtained her Doctor of Physical Therapy degree from Alabama State University prior to completing her Sports Physical Therapy Residency with Drayer Physical Therapy in 2016. Dr. Martin holds a board- certification in sports (SCS) and is a certified athletic trainer. In addition to treating patients in the Drayer Physical Therapy Institute in Birmingham, AL, Dr. Martin mentors in the sports residency program, provides new graduate physical therapist mentorship, and serves as a clinical instructor to physical therapy students. Her areas of special interest include concussions, female athletes, and overhead athletes.

Dr. Elizabeth Lane

Dr. Elizabeth Lane is a faculty member with URI's Orthopaedic Residency and continuing education programs. She earned her doctorate in physical therapy from the Medical College of Georgia and Ph.D. in Rehabilitation Sciences from the University of Utah. She holds board-certification in orthopaedics and is a fellow of the American Academy of Orthopaedic Manual Physical Therapists. She holds several certifications- dry needling, Selective Functional Movement Assessment, Certified Strength and Condition Specialist, and Therapeutic Pain Specialist. She has lectured at multiple national conferences and served on the Education Academy's Residency and Fellowship Education Special Interest Group. She currently serves as Assistant Research Faculty at the University of Utah DPT program. Her research interests include a focus on neuroscience education for those with chronic spinal pain.

Dr. Glenn Thompson

Dr. Glenn Thompson is an Upstream Rehab Institute instructor and mentor and a physical therapist practicing in Crossville, TN. He is an instructor for: MT2 and MT3 in the URI Orthopaedic Manual Physical Therapy Fellowship, musculoskeletal and dry needling courses in the URI Orthopaedic Residency and multiple continuing education programs. Dr. Thompson obtained his entry level physical therapy degree from East Tennessee State University and his Doctor of Physical Therapy from Evidence in Motion in 2014. He completed a fellowship in Orthopaedic Manual Physical Therapy with Evidence In Motion in 2017 and is recognized as a fellow of the American Academy of Orthopaedic Manual Physical Therapists. Dr. Thompson has also achieved board certification in orthopaedics (OCS). He is a clinical instructor and serves as a mentor orthopedic residents and fellows-in-training.

Dr. Grace Mollohan

Dr. Grace Mollohan is the Director of Orthopaedic Residency as well as a course instructor for Upstream Rehab Institute. She has been a practicing physical therapist in Atlanta, GA since 2011 when she obtained her Doctor of Physical Therapy degree from Emory University. Dr. Mollohan completed Orthopaedic Residency with Evidence in Motion in 2013 and completed OMPT Fellowship training with BenchMark Rehab Institute in 2018. She has achieved board certification in orthopaedics (OCS), and is recognized as a fellow of the American Academy of Orthopaedic Manual Physical Therapists. Dr. Mollohan is a clinical instructor and serves as a mentor to new graduate physical therapists, orthopedic residents and fellows-in-training. Dr. Keri Rogers

She has achieved board certification in orthopaedics (OCS), and is recognized as a fellow of the American Academy of Orthopaedic Manual Physical Therapists. Dr. Rogers is a long time clinical instructor and serves as a mentor to new graduate physical therapists, orthopedic residents and fellows-in-training. Dr. Rogers has also held roles as a teaching assistant and adjunct faculty at the University of Tennessee Chattanooga and Southern Adventist University. Dr. Mark Powers

Dr. Mark Powers is primary faculty with Upstream Rehab Institute's Orthopaedic Manual Physical Therapy Fellowship and a physical therapist/clinic director practicing in Savannah, GA. He is also an instructor for the URI Modern Pain Science course. Dr. Powers obtained his Doctor of Physical Therapy degree from Armstrong Atlantic State University. He has achieved board certification in orthopaedics (OCS), completed an Orthopaedic Manual Physical Therapy Fellowship program through Nxt Gen Institute of Physical Therapy, and is recognized as a fellow of the American Academy of Orthopaedic Manual Physical Therapists. Dr. Powers has previously published a narrative review looking at the role of mirror neurons and observation in rehabilitation and he has presented at regional conferences discussing the role of therapeutic neuroscience education and clinical language in treatment of chronic pain.

She has graduated from URI's Orthopaedic Residency and has achieved board certification in orthopaedics (OCS), and a certification in dry needling from two different programs. GREG LINDLEY

Dr. Lindley is Clinic Director for BenchMark Physical Therapy in Spring City, TN. He is on faculty for the URI Hand Therapy Fellowship. He works on course development and teaches the manual therapy lab for the Hand Therapy Fellowship. Dr. Lindley earned his B.S. in Health Promotion from Auburn University in 1991 and his M.S. in Physical Therapy from the University of Colorado Health Sciences Center in 1994. He earned his DPT in 2018 from the University of Montana. He achieved board certification as an Orthopedic Clinical Specialist in 2001. He became a Certified Hand Therapist in 2011.

Jennifer DeYoung

Jennifer DeYoung is the Director of Upstream Rehab Institute's Hand Therapy Fellowship and led its development beginning in 2016. She received her Masters of Occupational Therapy degree from Samuel Merritt University in Oakland, California, and has been a Certified Hand Therapist since 2002. Jennifer is a Certified AOTA Fieldwork Educator, an active member of the ASHT, AOTA, and has served as a board member of the Georgia Hand and Upper Extremity Special Interest Group (GHUESIG). She joined BenchMark Rehab Partners in 2012 and continues her practice at the Canton, GA clinic. Jennifer served as BenchMark Rehab Partner's Georgia Hand Therapy Coordinator from 2014-2015. Her additional experience includes lecturing nationally for DeRoyal's Orthotic Fitter course, earning a certification as a Kinesio Taping Practitioner, performing both corporate ergonomics and functional capacity evaluations, and working as a clinical specialist account manager for a DME company. Jennifer is primary faculty for URI's The Wrist Complex: Evidence-Based Concepts in Wrist Rehabilitation course, and led development of the Hand Therapy Bootcamp and Introduction to Orthoses courses.

Michael Robert Dunning

Michael Robert Dunning is currently the Clinical Director and Partner at Benchmark Physical Therapy in McDonough Georgia. He graduated from Valdosta State University in 2001 with a Bachelor's of Science Degree in Sports Medicine & Athletic Training, and currently holds a certification as a licensed Athletic Trainer. Michael continued his studies at Georgia State University to receive a Master's in Physical Therapy in 2004. Following school, he completed a Residency and Fellowship program with "Institute of Rehabilitation and Training" and obtained certification as a Fellow in the American Academy of Orthopedic Manual Physical Therapists in 2007.
