Adrienne Solis

Dr. Solis-Sherman joined Senior Care Partners PACE as Advance Care Planning Manager in April of 2023. She transitioned to Director of Program Integrity in December of 2023. Prior to joining SCPP, Adrienne served as a board member for Comprehensive Senior Care Corporation from 2018-2023. She worked as the Director of Home Care, Hospice, and the Wound Center at Oaklawn Hospital from 2016-2022 and has worked in a variety of inpatient and outpatient clinical nursing positions throughout her career. She has worked fervently in her leadership roles to improve the way we support our healthcare delivery teams and believes this plays an integral role in our ability to provide exceptional high-quality care. Adrienne started her nursing education journey at Kellogg Community College. She went on to earn her BSN and later her MSN with a Healthcare Systems Management concentration. She earned her Doctor of Nursing Practice degree at the University of Michigan, Ann Arbor. Adrienne's doctoral studies focused on implementing evidence-based practice and leading care delivery in complex settings across the care continuum. During her time at the University of Michigan she studied extensively reducing compassion fatigue and improving compassion satisfaction through trauma-informed self-care for healthcare professionals. She is passionate about serving the underserved and improving health equity. Adrienne also holds board certifications in Hospice and Palliative Care Nursing and Healthcare Quality.

Anita Grivins - Treasurer

Anita received her B.A in Accounting from Western Michigan University and went to work for Plante Moran, obtaining her CPA license there. Her focus was on the healthcare industry performing audits and consulting work for hospitals, long-term care facilities and continuing care retirement communities. Upon leaving Plante & Moran, she worked at the Pokagon Band of Potawatomi Indians for 10 years as their Director of Finance, overseeing the related finances for the government operations and assisted in the monitoring of gaming operations. Anita has over 30 years of experience with accounting, budgeting, forecasting, grant administration, audits, compliance, strategic planning and involvement with several Boards. Anita has remained very active in her Latvian community both here in the Kalamazoo area as well as on a national level.

Brenda Hunt

Brenda is a native of Michigan, raised in rural Montcalm County. She received both her undergraduate and graduate degrees from Western Michigan University. Brenda has 40 years of experience in the nonprofit sector, she joined the Battle Creek Community Foundation in 1993 as a program officer. She was promoted to vice president in 1995, to chief operating officer in 1997 and began as president & CEO in April of 1998. She was previously employed with the Greater Kalamazoo and Holland United Way's and the American Red Cross, Kalamazoo County Chapter. Brenda is known throughout the state for her collaborative approach to addressing community and regional needs. She has provided leadership and guidance in fostering public-private partnerships and has been instrumental in bringing about refocusing initiatives to be more collaborative and inclusive. During her tenure with the Battle Creek Community Foundation, Brenda initiated the Regional Health Alliance (RHA) a collaborative planning and programming initiative. Re-structured as the Population Health Alliance in 2019, focus areas have included access to care, infant mortality, obesity and teen pregnancy. Through collaboration and advocacy, the work of RHA attracted Western Michigan University Homer Stryker School of Medicine to Battle Creek to establish a family residency program. During the pandemic Brenda chaired a task force that created the Battle Creek Shelter, an independent nonprofit organization that serves as a 24/7 low barrier shelter for men and women. Brenda provided leadership for the acquisition and restructuring of the Burnham Brook Community Center, now, Kool Family Community Center, a title holding corporation of the Battle Creek Community Foundation. The restructuring of the Center established a consolidated home delivered meal kitchen/office for Calhoun County as well as the financial infrastructure required to make the facility the home of Senior Care Partners PACE. During her tenure in Battle Creek, Brenda has been involved with numerous efforts to increase access to health services. She served on the Board of Albion Hospital until a re-alignment with Jackson County occurred. In 2001 she was once again engaged in the effort to create the Albion Health Care Alliance (AHCA), an independent nonprofit organization that was created to provide access to health services after the hospital was closed in 2001. An effort where BCCF continues to provide fiduciary and management services to support AHCA's sustainability. Health related Board service includes eighteen years on the BBCH Community Partners Board of Directors, five years on Battle Creek Health System, and ten years on the Bronson Healthcare Board of Directors. Brenda has served on three search efforts to secure health care executives, numerous health system committees and task forces. She has attended five NRC Governance Institutes for board members. In her role as a local funder Brenda has directed or collaborated on both small and large health funding and financing efforts for 30 years. She has received several honors for her work including the Dale Griffin Health Leadership Award and the John and Kay Polzin Ambassador for Healthcare Award from Bronson Health Foundation. Brenda continues to serve as an advocate for health needs both in the community and board room.

Daniel Brauner

Daniel Brauner, MD, is a board-certified internist with added specializations in geriatrics, rheumatology, and medical ethics recently recruited as the Chief of the Division of Geriatrics at Western Michigan University Homer Stryker M.D. School of Medicine in Kalamazoo, Michigan. He received a bachelor of science in biology from Cornell University, Ithaca, New York, and his medical degree from Upstate Medical University, Syracuse, New York. He was a resident and chief resident at Cook County Hospital in Chicago, completed his Rheumatology Fellowship at the University of Illinois, Chicago, Medicine and Cook County Hospital, and a National Endowment for the Humanities Fellowship at the Institute for Humanities and Medicine. He was a founding member of the section of Geriatrics at the University of Illinois, Chicago, where he practiced rheumatology and geriatrics. He then moved to the University of Chicago, where he concentrated on geriatrics practice and medical ethics, was the director of the Geriatrics fellowship, medical director of its teaching nursing home, co-director of the ethics consultation service, and assistant director of the MacLean Center for Clinical Medical Ethics Center. His research interests include the historical genesis of current standards of care, the evolution of the language of medicine and its influences on care, how cognitive impairment influences care, and issues of access to care.

Daniel Pontius

Daniel Pontius is the Chief Executive Officer of MRC Industries where he works to encourage and support individuals living with disabilities to achieve their fullest potential. Prior to this position Dan served for 13 years ,in various roles, at Milestone Senior Services where he ended as the Chief Operations officer. He also serves as an Adjunct Professor in the Culinary Department at Kalamazoo Valley Community College. Educationally, Daniel earned a Bachelor of Science in Business Management and Organizational Development and a Master of Science in Management from Spring Arbor University. In addition to his work, Daniel serves on the Kalamazoo Valley Community College Culinary Advisory Council, the Bronson Healthy Living Campus Strategy Team, OnePlace Nonprofit Leadership Academy, the Fund Development Committee at Milestone Senior Services, and is a Certified ServSafe Instructor and Registered ServSafe Examination Proctor at ServSafe.

Dr. Rebecca Schreiner

Dr. Rebecca Schreiner has been practicing medicine in the Kalamazoo area since 2001. She began her career at Senior Care Partners P.A.C.E. as assistant medical director in January 2021 and promoted to medical director in November 2021. Board certified and residency trained in Internal Medicine/Pediatrics and Obesity Medicine with strong problem solving, communication, leadership and interpersonal skills, Dr. Schreiner possesses extensive clinical experience with strong skills in end of life/geriatric care and family conferences. She enjoys problem solving, creating thinking for positive change, and strategic planning. Dr. Schreiner earned her bachelor's degree in biology from Gonzaga University in Spokane, Washington, and her doctorate in medicine from the University of Washington in Seattle, Washington. She completed her residency and Chief resident training at Michigan State University-Kalamazoo Center for Medical Studies. Prior to joining the Senior Care Partners P.A.C.E. team, Dr. Schreiner was the Primary Care Lead Physician at Bronson Lifetime Wellness and a nocturnalist at Ascension Borgess Hospital and Bronson Children's Hospital. She is active in her home community as a mother of 5, youth sports coach, Girl Scouts of America leader, Comstock STEM Academy Advisory Board member, Comstock Public Schools Strategic Planning Committee member and as a Science Olympiad coach.

Erin Majka - Chairman

Erin Majka concentrates her practice in the areas of estate planning, estate administration, and long term care planning. Erin graduated from DePaul University College of Law (magna cum laude and Order of the Coif). Before attending law school, she graduated from DePauw University, which is located in Greencastle, Indiana. She is licensed to practice in both Illinois and Michigan. After law school, Erin was an attorney with a firm in Chicago for eleven years before moving home to Michigan. Erin is a board member for both the Hospital Hospitality House of Southwest Michigan, located in Kalamazoo and Senior Care Partners P.A.C.E - Program of All-Inclusive Care for the Elderly (PACE), located in Battle Creek and Kalamazoo. She is also a member of the Junior League of Kalamazoo. She enjoys spending time with family and friends, as well as reading and volunteering.

Karol Clason

Karol Clason oversees strategic planning and business development for Bronson Healthcare headquartered in Kalamazoo, Michigan. She has over 29 years of experience in health care and 22 years in strategic planning. Karol received her Bachelor and Master of Business Administration from Walsh College in Troy, Michigan. She served as a Senior Examiner for the Michigan Quality Council and is a Fellow with the American College of Healthcare Executives (FACHE). Karol also served six years on the Hospital Hospitality House of Southwest Michigan Board.

Katie Klute - CHRO

As a lifelong student of people and their relationship to their environments, Katie Klute is motivated to working collaboratively with others to create workplaces of accountability, psychological safety, and growth. Her internal drive to improve processes, develop trusting relationships, and think globally about the human experience, led her to a career in Human Resources. With over a decade of HR experience and a degree from Michigan State University in Family Community Services, Katie brings a solid foundation for HR leadership at Senior Care Partners P.A.C.E.. Katie spent time after college as a stay-at-home mom working part-time jobs in the non-profit sectors of healthcare and government prior to starting a career in HR in for-profit corporations. While working for a Kalamazoo-based advertising company, Katie learned the value of a human-centered approach in business from her direct supervisor and the company's owners. When she left advertising for manufacturing, she took those lessons with her and championed a new approach to HR that would transform the manufacturing experience for employees and leadership. During her time in manufacturing, she led talent and organizational development, recruiting, DEI, performance management, policy writing, and employee relations. In 2017 she was certified as a Gallup Engagement Champion and in 2018 as a Gallup Strengths Coach. In June of 2022, Katie earned her SHRM-SCP certification. Katie is continually inspired by the depth and resilience of the human spirit and will research and read anything that might improve her understanding of human nature and the industry she works in. Outside of work, Katie most enjoys reading, yoga, hiking, traveling, designing welcoming spaces, and hanging out with people she learns from.

Kelly McCormick

Kelly McCormick, MBA, BS, RN, is the Chief Executive Officer of Centrica™ Care Navigators. Prior to this position, she served the same organization as the Chief Operating Officer for a number of years where she provided overall direction and provision of the agency's clinical and operational areas including quality service delivery, strategic planning, business operations, census growth, compliance, and financial oversight. Prior to these positions, she served as Director of the Nursing Resource Team at Borgess Medical Center, Community Relations Officer and Director of Residential Services at Centrica™ Care Navigators, formerly Hospice Care of Southwest Michigan, Marketing Specialist at Borgess Health Alliance and Product Manager in Sales and Marketing at Stryker Medical. Kelly earned her Registered Nurse Diploma through the Bronson School of Nursing. She also holds a Bachelor of Science in Health Studies and a Master of Business Administration (MBA), both from Western Michigan University. She is nearing completion of a Master of Science in Nursing (MSN) with a concentration in Leadership and Management through Walden University where she is a member of the Walden University's International Nursing Honor Society which recognizes academic achievement and integrity. In addition to her work responsibilities, Kelly formerly served on the Advisory Committee for the Kalamazoo Community Foundation's Women's Education Coalition Grant, is the former Vice-chair of the Public, Community, and Member Relations Committee for the Michigan Hospice and Palliative Care Agency, and is a former Editorial Consultant for Home Health Aide Digest. Additionally, she has been a presenter at the Michigan Hospice and Palliative Care Agency Conference and has served on several boards and committees in the Kalamazoo area.

Laura Hamann

Laura Hamann, MBA, retired as VP of Post-Acute and Care Management at Bronson Healthcare Group where she gave executive oversight to their skilled nursing facility, Adult Illness Management inpatient and outpatient programs, home care, and durable medical equipment services. She also provides oversight for the care management of twenty-seven primary care practices. Prior to her current position Laura was the Assistant Vice President and Executive Director of Bronson Healthcare Group, the Vice President of Lakeview Hospital, and the Executive Director and Administrator of Bronson Healthcare Group. During her tenure as Assistant Vice President and Executive Director of Bronson Healthcare Group, the facility to which she gave oversight received the American College of Healthcare Administrators Silver National Quality Award (2018), Centers for Medicare and Medicaid Services 5 Star Performance Award for seven years, and was listed in the U.S. News and World Report Best Nursing Homes for seven consecutive years. Laura earned a Bachelor of Business Administration (BBA) from Western Michigan University and a Master of Business Administration (MBA) from the University of Notre Dame. She is a Licensed Nursing Home Administrator through the State of Michigan, a Fellow in the American College of Healthcare Administrators (FLNHA), and a long standing Health Care Association Board Member and Chair.

Matthew R. Cooper

After graduating from Valparaiso University School of Law, with honor awards in contracts and criminal law, Matt clerked for a Circuit Court Judge. Matt then joined the Law Offices of Schuitmaker, Cooper, Cypher & Knotek, P.C. Over the last 25 years, Matt has represented clients in major litigation cases and has participated in jury trials in Van Buren, Kalamazoo, Barry and Wayne Counties. Further, Matt has prosecuted cases in Illinois, Maryland, Indiana and Michigan at the Federal and State Court levels. Matt has worked on both civil and criminal cases. Matt has appeared in Circuit Court in over seventeen (17) Michigan Counties. Matt has also handled cases in the Western and Eastern Federal District Courts, the United States 6th Circuit Court of Appeals, the Michigan Court of Appeals and the Michigan Supreme Court. In 2011, Matt was selected by the Paw Paw Chamber of Commerce as the Community Volunteer of the Year. He is a charter member and past President of the Paw Paw Area Rotary Club. Matt is past president of the Optimist Club. For numerous years, he has worked for Paw Paw Rocket Football, Inc., and Paw Paw Youth Baseball, Inc. Matt formed both of these organizations into Michigan Corporations and obtained their 501(c)(3) charitable organization status from the Internal Revenue Service.

Matthew Witt

Bringing with him a wide range of experience in his career as well as personal life, Matthew Witt, CPA, CTP, MCSA, joins Senior Care Partners P.A.C.E. as director of finance & administration beginning September 2021. Prior to SCPP, Matt was planning & analysis manager at Gun Lake Casino in Wayland, Michigan, where he built and managed annual budget and quarterly financial review presentations for the board, developed driver-based budgets and summaries for three new restaurants, and led the competitive analysis, economic market study, and board presentation of a proposed hotel expansion. Matt designed and led the roll-out of new pay ranges, bonus, and tenure calculations, increasing talent attraction and retention, reduced absences, and improved revenue and engagement. Earlier, he worked in public accounting at Deloitte & BDO, where he audited Henry Ford Health Systems and Pennock Hospital and other profit and not-for-profits. Matt has also worked at Denso, Perrigo, and Meijer, championing major improvements in automation and analyses within existing systems. He earned his bachelor's degree in finance and economics at Baylor University as a varsity athlete, and studied accounting at Grand Valley State University., In his personal life, Matt is a pilot, traveling to more than 40 countries. He is also a certified scuba diver. Matt served in the Army National Guard and U.S. Coast Guard.

Qiana Harrison - COO

Qiana Harrison is known for thinking "out of the box," making her a perfect fit for Senior Care Partners P.A.C.E. Her innovative approach to interdisciplinary care along with her ability to foster meaningful connections with team members and guidance for their career development has earned her the position of Chief Operating Officer. Qiana began her career with Senior Care Partners P.A.C.E. in June 2020 as a Community Supports Leader. By October 2020, she was promoted to Clinic Department Leaders. Qiana went on to serve as Director of Interdisciplinary Team Operations beginning in August 2021 and Senior Director of Operations beginning in March 2022. She was promoted to Chief Operating Officer in July 2022. Qiana earned her Bachelor's Degree in Healthcare Administration from Western Governors University. She is currently working on her MBA in Healthcare Administration from LSU. Qiana serves on the Palliative Care and End of Life Workgroup for the National P.A.C.E. Association (NPA), for which serves as chair. She is also chair of the NPA Place of Death Taskforce. With more than 25 years in health care, Qiana has worked in the private sector in surgical practice and more recently for United HealthCare in leadership and at P.A.C.E. of Southwest Michigan in clinical operations. She also owns and operates a family business with her husband.

Sally Berglin - CEO

Sally Berglin joined Senior Care Partners P.A.C.E. as chief executive officer November 2021. Prior to coming to SCPP, she had been the chief executive officer of Bronson LakeView Hospital in Paw Paw, Michigan, a 35-bed critical access hospital, a 100-bed long-term care facility, and advanced life support ambulance service. She has also served as vice president of Bronson Healthcare Group. She previously served in many roles at Bronson Methodist Hospital including director of neuro, ortho, and trauma services and director of emergency services, a Level One Trauma Center. Sally earned her master's degree in public administration with an emphasis on non-profit organizations at Grand Valley University in Grand Rapids, Michigan, where she graduated summa cum laude. She earned her bachelor's degree in business administration, magna cum laude, at Nazareth College, and an associate degree in nursing at Kalamazoo Valley Community College in Kalamazoo, Michigan. Always an active member of her community, Sally has served on numerous boards, among which are MHA (Michigan Health & Hospital Association) Board of Directors; chair of the MHA Small & Rural Hospital Council; chair of the Van Buren County United Way Board; chair of the Van Buren Emergency Medical Services Ambulance Company; the YWCA Board of Directors; Mattawan Public Education Foundation Board of Directors; and many more.
