OAK VILLAGE

As a resident moves into the community, we work closely with their loved ones to perform a customized assessment so an individual care plan can be created. We want to get to know each of our residents as the unique individuals that they are. Our care plans are updated every six months or as needed. Led by the Medical Director, our community is staffed by highly trained RNs, LPNs, and CNAs. They are hired not only for their professional skills but also for their compassionate and warm personalities... As a senior housing industry participant and regional leader, we are enthusiastic about the future of our field and are eager to assist companies in meeting their stated goals and objectives...
