San Diego PACE is unique because it offers a comprehensive and specialized health care clinic which is integrated with a PACE Adult Day Center and Rehabilitation Therapy specifically tailored to meet the needs of older adults. San Diego PACE also offers home care services to those who are eligible as well as free transportation services to the Day Center and all authorized appointments.
