MAIMONIDES MEDICAL CENTER

New York State has worked with the Federal Government to initiate the Delivery System Reform Incentive Payment (DSRIP) program, which will allow the state to reinvest over six billion dollars to improve healthcare for Medicaid beneficiaries and reduce avoidable hospital use by 25 percent over the next five years. Maimonides Medical Center, building upon a strong foundation and deep experience in developing and implementing collaborative care models through prior state and federal programs, is poised to transform the healthcare delivery system in Brooklyn. Maimonides has teamed up with hundreds of provider organizations and thousands of physicians to establish Community Care of Brooklyn (CCB), a new model of coordinated care for Brooklyn's Medicaid population.
