CommunityHealth is one of the largest volunteer-based health centers in the nation. We provide primary and specialty care, medications, lab testing, mental health services, and health education at no charge to low-income, uninsured and underinsured adults in Chicago... Today, we have built a model that brings together volunteers, donors, and community partners, to serve thousands of Chicago's uninsured adults six days a week in a Patient-Centered Medical Home setting (recognized by the National Committee for Quality Assurance).
