The OHT-N is committed to meaningful community engagement, as well as partnership with patients and caregivers, to shape health and community care service delivery in Northumberland... Our main target audience represents adult adventurers, fun families, and retired roamers within the geographic area of the Golden Horseshoe/GTA, southern Ontario, eastern Canada, bordering the US... Joined by patient and caregiver representatives, we are now moving forward with energy and enthusiasm to determine how we can improve the patient and family experience of health care in our community through the Ontario Health Team (OHT) model.
