The MINT Memory Clinic model is built on a strong foundation of research evidence and best practices, integrating standardized, nationally accredited training. Our model enables close partnerships between primary care, specialist care and community service agencies. A rapidly growing network of MINT Memory Clinics is spreading across Canada!... From the first referral to follow-up, MINT Memory Clinic teams work within a shared care approach with the patient's family physician. Patients with memory concerns are referred by their family physician to a MINT Memory Clinic for comprehensive assessment and care planning. The MINT Memory Clinic provides a detailed and proactive care plan to the family physician, and refers to specialists when necessary.
