Horizon Multicare Group is a group of passionate and proficient physicians who aim to bring the highest quality primary care to the Antelope Valley. We first began serving the local community at the Antelope Valley Medical Center (AVMC), caring for patients during some of the most challenging times of their lives. We are board certified in internal medicine and family medicine and we are able to deal with simple pneumonia to multi-organ failures, and our doctors are available in the hospital 24/7 to deal with any complications that may arise during your hospitalization. Horizon Multicare Group is also the only post-acute care physician group in the Antelope Valley, and we are ready to provide care in the nursing home as well as in the outpatient setting through our affiliation with Unified Health Associates.
