This is a fee for service practice. Payment is required at the time of service by cash, check or credit card. Claims are filed electronically or by the patient depending on the insurance. Claims are not sent to Medicare for the office visit as this is prohibited by statute for non contract physicians. Diagnostic testing performed by outside vendors is generally covered directly by private insurance and Medicare. Certain Medicare Advantage, PPO and HMO plans may require the contract primary care provider to write orders for diagnostic testing. Dr. McCullars provides these practitioners with the necessary testing information.
