Alexander Pujol

BIO Alexander Pujol is a board certified physician assistant graduating from the prestigious program at Nova Southeastern University in 2000. With nearly 20 years of clinical experience helping patients in chronic pain management, Alex, has developed a more holistic approach to patient care. Through his previous experience, he has garnered the respect of many primary care physicians and specialists in the local community. Alex has participated as a sub-investigator in many research projects through the years primarily during his tenure with the Institute of Pain Management. Being fluent in English and Spanish, he provides greater access to a more diverse population. Mr. Pujol is also board certified in general surgery as a physician assistant. Alex demonstrates expertise in many areas of chronic pain with emphasis in patient selection for advanced therapies, to include but not limited to, peripheral stimulation, spinal cord stimulation, intrathecal pump management, neuraxial spinal procedures and peripheral nerve blocks. Mr. Pujol's practice beliefs and values fall in line with that of the physicians at Formoso Spine & Joint Pain Specialists. He values practicing pain medicine with a comprehensive care model by using behavioral techniques, conservative medical treatment along with recommending evidence based interventional pain management procedures. Ashley Schivelbein

Ashley Schivelbein is a board certified advanced registered nurse practitioner. She received her Bachelor of Science in in Psychobiology and a Minor in Chemistry and graduated with honors from Simmons University in Boston. She earned her Master's in Science degree in Family Health Nursing from Boston College. Prior to working at Formoso Pain Specialists, she worked at Mayo Clinic in the Endocrine department managing all Endocrine conditions and diabetic nerve pain. Ashley has over 8 years of experience working in primary care and managing all types of pain including nerve, musculoskeletal and chronic pain syndrome. In her free time, she enjoys spending time with her family at the beach as well as traveling.

Dr. Ferdinand Formoso

Dr. Ferdinand Formoso is a board certified interventional pain medicine specialist as well as a board certified physical medicine and rehabilitation specialist. After completing his bachelor's degree in Psychology from the University of Buffalo in 1996, he earned his doctorate degree at the New York College of Osteopathic Medicine in 2001. Dr. Formoso was chief resident for the Temple University/Moss Rehab pain management and rehabilitation residency program in Philadelphia. Under Temple's Program Director Dr. Frank Falco, a past President of the American Society of Interventional Pain Physicians, he completed his fellowship in interventional pain medicine. Dr. Formoso is a founder of FORMOSO Center and serves acute and chronic pain and rehabilitation patients in Jacksonville.

Lee Irwin

Dr. Irwin is board certified in pain management, physical medicine and rehabilitation. He earned his medical degree from Temple University School of Medicine in 2000. He also holds a Bachelor of Science in chemistry and a Bachelor of Science in biological science. He completed his internship in preliminary medicine at the Reading Hospital & Medical Center. Soon after he completed his residency at Temple University, Dr. Irwin completed his fellowship in pain medicine at Temple University Mid-Atlantic Spine Pain Medicine as an ACGME accredited fellowship. He has also completed the Helms Acupuncture Course at UCLA. Prior to joining Coastal Spine and Pain Center, Dr. Irwin was the Program Director at Loma Linda University Pain Medicine Fellowship Program, the Medical Director for the Center for Pain Management at Loma Linda University School of Medicine, the Assistant Professor of Medicine in the Department of Physical Medicine & Rehabilitation at Loma Linda University School of Medicine. Dr. Irwin has also been honored with an award in Excellence in Research in 1994 by the Department of Biological Sciences at the University of California, Irvine. He has also taken part in multiple publications and presentations.

Veronica Antoine

Veronica Antoine, M.D. is board certified by the American Board of Anesthesiology. She earned her doctorate degree from the University of North Carolina in 1992. Dr. Antoine's medicine residency was at Johns Hopkins University School of Medicine, in the Department of Anesthesiology and Critical Care Medicine. She also completed her fellowship at John Hopkins, in the Division of Pain Medicine, Department of Anesthesiology & Critical Care Medicine. Before joining Coastal Spine and Pain Center, Dr. Antoine was the Pain Medicine Specialist and Coordinator of Inpatient Pain Services at the Kaiser Medical Group in Honolulu, Hawaii. She was the Director of Pain Medicine Services in the Department of Anesthesia at Georgetown University Medical Center, Washington, DC. Dr. Antoine has chaired and co-chaired multiple committees, including serving as chair of the Inpatient Interdisciplinary Pain Workgroup in the Department of Neurosciences-Pain Medicine Division and co-chair of the Health and Wellness and Well-being Committee for Kaiser Hawaii Medical Group. She also served as Kaiser's Physician Advisor to the Pain Relief Nurse Champion (PRNC) program. Dr. Antoine instructed medical students in the Department of Anesthesia at Georgetown University. She has published extensively in respected publications, and has received multiple awards, including Hawaii's "Top Doctors" and Washingtonian's "Top Doctors" in Pain Management. She is a member of a number of medical associations, including American Society of Regional Anesthesia and Pain Medicine, National Medical Association and American Pain Society.
