LTSS CARE PARTNERS

As a Community Partner program funded by MassHealth, we partner with MassHealth Members to improve their access to healthcare through identifying and coordinating a wide range of the services and supports they need. We work with other healthcare and social services providers to coordinate and integrate care that promotes improved health and wellness... As both a BH and LTSS CP, we are a collaboration of four long standing, community-based organizations - Bay Cove, Justice Resource Institute, Mystic Valley Elder Services, and Vinfen. Our multidisciplinary care teams are comprised of Licensed Social Workers, Community Health Workers, Registered Nurses (RNs), and Recovery Coaches... Our LTSS CP serves MassHealth Members in the greater Boston area, as well as Malden. Our BH CP serves MassHealth Members in eastern Massachusetts from Lowell and Lawrence to Fall River and Cape Cod. Our expansive service area promotes partnerships with 14 ACOs and their primary care practices.
