This service will be person centered and the treatment will be dictated based upon the needs assessment of the consumer. In addition, this service will focus and be responsive to cultural, racial and ethnic differences of the population we serve... We provide Medicaid Targeted Case Management services to at-risk children, adults with serious and persistent mental illness, at-risk pregnant women and infants and individuals with psychoactive substance disorder.
