Working collaboratively with other professionals, we provide essential non-clinical support immediately post hospital discharge as well as a more long-term ongoing support... Our Reablement Support Workers (RSW's) are carefully selected, police checked, have completed full induction training, Brain Injury specific training, additional specific training related to individual conditions and further 'shadowing' with other Support Workers before working with our clients. They have full instructions for each clients outcomes in the Client's Care Plan and, our Local Office staff are available to support them at all times...
