Since 2012, the Heart of Georgia Transition Alliance has supported local school districts in providing training, support, community awareness, and student related activities to help increase post-school outcomes for students with disabilities... The Heart of Georgia Transition Alliance seeks to improve transition services, transition support, and post-secondary outcomes for students with disabilities.
