Eric Bouwman

Eric is the Vice President of Operations for Bloom Hospice. Eric is passionate about integrated care and develops operational strategies, efficiencies, and systems to enhance our patients' hospice experience. Eric has a decade of healthcare and medical social work leadership experience, beginning his career as a clinical social worker and was most recently a Regional Director at Kaiser Permanente before joining Bloom. Eric received an MBA from Colorado State University as well as a Master's in Social Work from Denver University. A Michigan native, he attended Hope College for his undergraduate degree. Outside of work, Eric enjoys spending time with his wife and two kids as well as biking, running, and skiing.

Hope Bruner

With a solid decade of unwavering dedication to hospice and end-of-life care, Hope Bruner brings a depth of expertise and passion to her role as Clinical Supervisor at Bloom Healthcare. Her commitment to patient-centered care and professional growth has made her an influential figure in the realm of healthcare, especially within the Colorado region. Before her pivotal role at Bloom Healthcare, Hope was the Clinical Manager of an Inpatient Hospice Unit, where she showcased her leadership skills and dedication to providing unparalleled patient care. Under her guidance, the unit flourished, setting benchmarks for quality and compassionate care. Her roles have spanned from Director of Quality, Compliance and Education for Hospice and Home Health to Interim Director of Clinical Services, showcasing her versatility and breadth of knowledge in the field. Throughout her illustrious career, Hope has held various roles, encompassing quality assurance, patient care, and administrative responsibilities. This diverse experience has enriched her with a holistic understanding of hospice care operations, federal and state compliance regulations, and the nuances of patient-centered healthcare. Outside the confines of her professional life, Hope is deeply connected to nature, having been born and raised in Montana. Jaime Stephenson

Jaime brings a decade of hospice experience to her role as Clinical Education Quality Manager where she oversees the onboarding and continuing education for new clinical staff as well as working to ensure Bloom's care exceeds clinical standards. Jaime started her hospice career as an RN Case Manager and was most recently a Clinical Services Director before joining Bloom Hospice. Outside the office, Jaime loves spending time with her husband and two kids while trying to find time to mountain bike and ski.

John Freeman - President

John Freeman, a distinguished leader in healthcare, brings over two decades of specialized experience in end-of-life care to his role as a Board Member and Co-Founder of Bloom Healthcare. His transformative leadership has been recognized across both local and national healthcare landscapes, significantly influencing some of the country's most reputable healthcare services. Before co-founding Bloom Healthcare, John served as the CEO of Honor Hospice, a Colorado-based, award-winning end-of-life care provider. Under his leadership, Honor Hospice not only became a market leader but also received the "Top Workplace" award from the Denver Post in 2015 and 2016. His successful tenure culminated in the acquisition of Honor Hospice by a Fortune 100 company in 2016. Throughout his extensive career, John has held a range of roles that included Regional and National Sales, as well as startup and Operational oversight. This broad experience has provided him with a comprehensive understanding of healthcare de novo and acquisitions, organizational development, and federal and state compliance regulations. He is particularly renowned for creating work cultures that equally prioritize employee well-being and exceptional patient care. Outside of his professional commitments, John enjoys traveling with his wife and children, running with his three dogs, and indulging in rounds of golf. John holds an active Hospice Operator license through CHAP (Community Health Accreditation Partner), reaffirming his unwavering commitment to maintaining the highest standards of healthcare excellence.

Sonia Fossen

Sonia Fossen's career has been marked by her passion for nursing, teamwork, and innovation. For nearly three decades she has focused on uniting hospice business operations and clinical practice to create models that meet the needs of today's businesses, employees and clients. Sonia is skilled in establishing and retaining strategic partnerships with key stakeholders to create new service opportunities. She recognizes and integrates the science and art of professional nursing care with business practices in the expanding healthcare regulatory environment. Sonia started her career as a nurse at Hospice of St. John in Lakewood, CO. After three years, she was promoted to Director of Clinical Services and served in that capacity for seven years. In that role, she oversaw all aspects of clinical operations in homecare, general inpatient and in the residential setting while significantly growing the program within a thirty-six-month period. She participated in community outreach efforts as an ambassador for Colorado Hospice Organization (now the Hospice & Palliative Care Association of the Rockies) and focused on rural program service delivery challenges and collaborative solutions. She also facilitated a statewide hospice program through Colorado Hospice Organization designed to connect urban hospices with resources to assist rural hospices in need. Sonia was integrally involved in the startup of Halcyon Hospice & Palliative Care, established in 2006. She served as co-founder, Director of Clinical Services, and Vice President during her 12-year tenure. She facilitated the program and service development in all facets for a startup certified hospice provider, overseeing all clinical operations in the home-based and inpatient settings. She led the organization's regulatory compliance program culminating in achieving Joint Commission accreditation and continued recertification status. Sonia initiated several community outreach programs aimed at supporting patients and families with limited access to quality end of life care in urban, suburban, and rural settings. Under her clinical leadership, she led the development of a home-based palliative care program. She developed innovative outreach services to reach community members, including a Blessing of the Hands memorial service incorporating aspects of Indian spirituality and bereavement practices that has touched thousands of lives across Colorado. Sonia understands how to assess and streamline business operations while engaging employees to achieve company objectives. She is an accomplished team leader who embraces mentoring of new clinicians. She has integrated communities and workplaces with a foundation of diversity awareness and inclusion. Sonia received her master's degree in Nursing with an emphasis in leadership from Regis University in Denver, CO. She has her associate degree in nursing from Arapahoe Community College in Littleton, CO. She is a member of Sigma Theta Tau International. Sonia achieved her Certified Hospice & Palliative Nurse designation through the Hospice and Palliative Nurses Association where she is also a member. She has been active in both state and national hospice and palliative care organizations. Sonia is also a Certified Test Observer for State of Colorado.
