SNF/LTC is provider owned and operated - all profit goes to members, not to a CEO. The co-op also supports and encourages independent practitioner status for NPs in ALF and Long Term Care... We are a small group of providers chosen for their experience in long term care. Our providers all have at least 3 years' experience in working in long term care, some of our providers have over 20 years... SNF/LTC Partners formed in 2019 with the aim of providing high quality, on-site care for elders in Assisted Living, Long Term Care and Skilled Rehab. Our group of geriatric specialists is small and selective; our focus is on providing extended presence in facilities and expert care to complex patients.
