Keys2Memory® is a Nurse Practitioner owned private medical practice in Mystic, Connecticut and delivers a house call 'focused neurocognitive' clinical evaluation, problem identification and strategies. My practice cares for a diversity of clients with memory loss, declining problem solving, self-neglect, apathy, and many other symptoms of dementia... A house call strategy allows a comfortable setting where patients perform their best, home safety can be evaluated, trust initiated, and a personalized care plan is devised. Our care model empowers you to learn, adapt and self-manage in a setting of privacy and partnership.
