INTERCEDE HEALTH

Intercede Health was founded by physicians in 1995. In that same year, we partnered with a Texas based not for profit Health System to redesign one of their Hospital's delivery of care models. By mid 1996, a remarkable turnaround had taken place resulting in a return to profitability for both the Hospital and its Health Plan partner, along with top decile clinical outcomes ranking. In the decades that followed, Intercede Health has provided Hospital Care Services in programs across the United States, with existing programs in over 40 hospitals today. In 1998, Intercede Health launched the Transitional Care Program, opening its first Transitional Care Clinic (TCC), as an alternative for patients at risk of hospital admissions or frequent use of emergency rooms. Since then, we have expanded our base of clinics into multiple markets, offering an effective way to treat chronic care or high-acuity patients in an outpatient setting.
