By entering into the Membership Agreement, you acknowledge that Cascade DPC does not provide health insurance coverage and this is not a contract for insurance. Cascade DPC provides only the Health Care Services specifically described herein in the covered services under the membership tab above and additional costs may be incurred for laboratory, medical imaging (at a Discounted Rate). Surgery, specialist care, emergency department visits, and hospitalization required are outside of Cascade DPC's services are not covered.
