ChronicDoc is a web and app based tool, facilitating seamless communication between the patient, their treating practitioner, the pathology lab and the pharmacy. Our program is based on an existing doctor-patient relationship and is suitable for those patients with chronic, controlled medical conditions... ChronicDoc is an application, allowing doctors and their patients to manage chronic medical conditions simply and conveniently... ChronicDoc works on a tier system based on the amount of patients a doctor services through the system. We currently offer packages for single-doctor practices and group practices with the range of accessible features also varying per tier.
