Bahar Ahmadzai

Bahar Ahmadzai has been working for the Concussion and Migraine Clinic/Hyde Street Community Services since 2021. She is currently the HR Manager/Executive Assistant to the CEO. She currently holds her Bachelor of Science from California State University Sacramento and is currently completing her Master of Healthcare Administration concentrating on Healthcare Management.

Beth Wendle

Beth joined the HSCS Board in 2023 and currently serves as board co-treasurer. She is in-house counsel at Lyft, a rideshare platform. Beth holds a J.D. from Loyola Law School, Los Angeles and a B.A. in political science from the University of California, Santa Barbara.

Dana Long

Dana joined the HSCS Board in 2023 and currently serves as board co-treasurer. She began her career as a healthcare consultant and currently works in Mergers & Acquisitions at Cisco. She also has experience working at and advising technology startups. Dana holds a B.A. from the University of Pennsylvania.

Diane Kostka

Diane joined the HSCS Board in 2023. She currently leads a technical solutions team at Sisense. Her prior experience involves designing EMRs, working with healthcare providers and insurance companies to optimize operational processes and using insight from healthcare data to drive patient outcomes.

Dr. Joanne P. Azulay

Dr. Joanne P. Azulay is a fellowship trained Neuropsychologist. Dr. Azulay obtained her PhD in clinical and Clinical Psychology with a specialization in Neuropsychology at Seton Hall University. She completed her American Psychological Association approved internship/residency at Robert Wood Johnson Medical School in both Neuropsychology and Psychology. Dr. Azulay completed her APPCN postdoctoral fellowship in adult Neuropsychology at JFK Medical Center in Edison, New Jersey. Since then she has been a practicing Neuropsychologist for the past 20 years. Dr. Azulay's areas of expertise are in clinical Neuropsychology, Cognitive Rehabilitation, Psychological Health, and Cognitive Restorative Mindfulness: A program she has pioneered at JFK's Center for Brain Injuries for the last decade for restorative cognitive functioning called the MAP (Mindfulness Attention Program) with great success. She currently serves as the Executive Director of Hyde Street Community Services in San Francisco. Previously, she served as the Director of Neuropsychology Program Development for the United States Air Force at Travis AFB in California. Dr. Azulay continues to serve as an Advisory Council Member to Governor of New Mexico. She is an Associate Professor of Neuroscience and Rehabilitation Medicine at Seton Hall University.

Emily Duran

Emily joined the board in 2019 and currently serves as board co-treasurer. She is a financial advisor at Seven Post Investment Office. She has experience in socially responsible investing, portfolio management, asset allocation, and risk mitigation. She received her M.A. from Johns Hopkins University and a B.A. from UC Santa Barbara.

George Swearington

George Swearington is the author of Side Sleeper Syndrome and the founder of HMD therapy for the correction of migraine headaches and has been a provider in the medical health field for over twenty years, practicing the art of homeopathy and the understanding of chronic pain conditions. A medical educator, clinician, and researcher, Mr. Swearington understands the plight of healthcare professionals addressing chronic pain conditions in the 21st century. Over the last decade, he has been both a provider and consultant for other professionals in the healthcare industry and lectured nationally and internationally on chronic conditions and their resolution. Swearington has utilized a powerful and innovative approach to applying his six foundational principles. Swearington's artful knowledge of homeopathy and groundbreaking views of cause and effect blends a beautiful consultative approach to retrieving the elusive answers to many of our medical questions.

Jason Finau

Jason joined the board in 2021 and currently serves as Board President. He is a licensed clinical social worker and currently serves as a supervisor with the Office of the Public Conservator in the city and county of San Francisco. Jason earned his M.S.W. from NYU, his B.A. in sociology from California State University San Marcos, and his A.A. in Behavioral Health Sciences from MiraCosta Community College.

Julia Cutts

Julia joined the board in 2017 and currently serves as the board Vice President. She is a social worker for the city and county of San Francisco. While obtaining her M.S.W. at San Francisco State, Julia was the first student from her program to claim a coveted position as a therapist trainee at Concussion and Migraine Clinic.

Kelsey Mrkonic

Kelsey joined the board of directors in 2021 and currently serves as board Secretary. She is a product manager at Juni Learning, an online computer science, mathematics, and English academy that offers private and online courses for students from ages 8-18. Kelsey holds an M.B.A. from Northwestern University's Kellogg School of Management and an undergraduate psychology degree from Stanford University.

Lauren Small

Lauren joined the board in 2021. She began her career working in corporate finance as a business analytics specialist. Personal life events led her to change career paths to pursue her passion for providing nutrition and wellness education to local communities. Lauren holds a B.A. in economics and a minor in Latin American studies from UC San Diego.

Manny Valentina

Manny Valentina graduated from the University of California, Merced with a BA of Psychology. Manny is a mental health worker at the Hyde Street Community Services (HSCS) and admin support at the Concussion and Migraine Clinic (CMC) since May 2021.

Sandra Lauer

Sandra joined the board in 2006. As a patient, she was involved with The Tenderloin Clinic in 1995, where she met the founding executive director, Cindy Gyori. She credits Cindy for turning her life around and developing her into an active volunteer, facilitating the organization's various socialization programs once a week. Sandra has also been a peer counselor and self-help specialist at San Francisco General Hospital since 1999.

Whitney Sunseri

Whitney joined the HSCS Board in 2023. She currently works for a local health system leading internal communications for all physicians, nurses, clinical and support staff, and volunteers. She received her M.P.S. from Penn State University and a B.A. from California State University, Chico.

Yvette D. Hunt

Yvette D. Hunt is the Operations Administrator at Hyde Street Community Services. I am also the Clinical Coordinator for the Concussion and Migraine Clinic. I have been a part of the organization since April of 2022. I am responsible for scheduling the providers in CMC, as well as completing the intake process with clients. I have a long history with the City and County of San Francisco DPH.(Population Health Division),where I was the Operations Administrator in the city. I have over 5 years of experience working with non-profit organizations and diverse populations.
