We are a Family Health Team comprised of Physicians, Physician Assistants, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians, Pharmacists, a Chiropodist, and other health professionals working together to provide primary and collaborative health care to the County of Brant and other surrounding communities. Through the Quality Innovation and Improvement Program we are involved in providing in-depth chronic disease management strategies... We offer a free, community-wide blood pressure clinic to promote cardiovascular health in our community and raise awareness about the importance of blood pressure monitoring. No appointment or referral is required. A copy of your results will be provided to you.
